Medically-Tailored Meals (MTM) Program

The Maryland Department of Health’s Medically Tailored Meals (MTM) program is a free initiative that provides personalized, home-delivered meals to help participants manage their diabetes. 

How it works:

People enrolled in the program receive the following at no cost, delivered to their homes each week:

  • 12 frozen medically tailored entrees per week, for six months
  • Additional fresh or frozen produce, as needed
  • Oral nutrition supplements, if and when prescribed by Registered Dietitian Nutritionists

Meals are provided to Allegany County residents by Food & Friends.

Participants are also offered ongoing nutrition counseling and assessment during the service period by a Registered Dietitian Nutritionist (RDN) licensed to practice in the State of Maryland. Meals will also be provided for any minor dependents (children) in the household of the participant.

I am an Allegany resident. Am I eligible?

Allegany County residents are eligible to receive meals if they meet at least one of the following requirements:

  • They visited the hospital two or more times during the past year because of diabetes, or
  • They had a diabetes Prevention Quality Indicator (PQI) event during the past year. These events include:
    • Diabetic coma
    • Diabetic ketoacidosis
    • Kidney, eye, or neurological complications caused by diabetes
    • Amputations related to diabetes
    • Hyperosmolarity

Taking part in programs such as WIC or SNAP does not affect your eligibility.

Your health insurance status and income also do not affect your eligibility.

How To Enroll: Patients

There are two ways to enroll in the MTM program.

  • Option 1: Reach out to your provider (doctor, nurse, social worker) to determine your eligibility and be referred to the program.
  • Option 2: If you think you may be eligible and do not have a provider who can refer you to the program, you can reach out directly to our partners:
