Healthy Heart Ambassadors: Blood Pressure Self-Monitoring Program

 

Blood pressure cuff, notebook, and pen.Have you been told you have high blood pressure (also known as hypertension)?

Do you take medicine for high blood pressure?

If you answered yes to either of these questions, you may qualify for a FREE program at the Allegany County Health Department that can help you take control of your health, reduce your blood pressure, and help prevent heart disease.

The Healthy Heart Ambassador Program is a 4-month program that can:

  • Train you in how to measure and track your blood pressure at home (if you do not have a blood pressure monitor at home, one will be given to you for FREE)
  • Teach you about nutrition and how to make healthier food choices to support lowering your blood pressure and maintaining a healthy heart
  • Provide you with the support you need from a trained lifestyle coach so that you have the best chance of lowering your blood pressure
  • Provide you with the tools you need to communicate your blood pressure results to your healthcare provider

To be eligible for this program, you must:

  • Be 18 years old or older
  • Have been diagnosed with high blood pressure, or be taking medications for your blood pressure
  • Not have had any cardiac event (heart attack or stroke) within the last year
  • Not have atrial fibrillation or other heart arrythmias
  • Not have lymphedema, or not be at risk for lymphedema (swelling in your limbs often caused by cancer or other conditions)
  • Be willing to meet with a lifestyle coach (virtually or in person) every other week for 4 months, and attend monthly nutrition sessions
